What Is GAPP?
GAPP Approved Private Care Agency:
Gapp is the Georgia pediatric program covering Medicaid services that provides skilled and personal care services to medically fragile children under the the age of 21. The service is completely free and NO COST AT ALL to the family. If your family is eligible for Medicaid you can apply for GAPP and get approved based on your child’s needs.

How it works
Approval Process
There is No Waitlist. Your Child Will Receive Services Soon As Your Approved Through Gapp. The Evaluation Takes Typically 30 Days Or Less
1st step: Intake Call
Where We Gather All The Information About Your Child Needs. From There We Schedule an in Home Meeting So the Family Can Get to know us and we develop a plan.
2nd Step: Care Plan
Based On All the Information We Gathered, We Come up with the Plan That We Feels fit the need for your child and family. We than review everything together to make sure everyone is on the same page. We than submit all the paperwork to GAPP.
3rd step: GAPP Decision
Heartfelt Comfort Care will receive a decision from GAPP and we will contact you to match you with a caregiver we both feel fits your family needs or if your denied we will discuss the appeal process.
